What services are included in my annual fee?

What is the mission of this practice?

Our mission is to deliver the highest standard of medical care, emphasizing proactive, comprehensive health management and disease prevention. In a professional yet relaxed environment, we prioritize preventing illness and optimizing your overall health and well-being. Your satisfaction with every aspect of your care is our highest priority.

How is this practice different from others?

Our concierge model allows for 30-minute routine appointments and 60-minute annual exams, ensuring personalized attention, minimal wait times, and enhanced coordination of your care.

What hospitals are you affiliated with?

We are affiliated with the WellStar Health System, including Kennestone Hospital. However, you are free to go to any hospital of your choice. We will work closely with hospitalists and specialists to ensure a smooth transition of care.

Where is your office located?

Our office is currently located at 1100 Johnson Ferry Road NE, Suite 190, Sandy Springs, GA 30342. PCCA has plans to relocate to the Cumberland / Battery District in Spring 2025; specifically, 3225 Cumberland Boulevard, Suite 550, Atlanta, GA 30339.

Who covers for you when you are unavailable?

We strive to be available 24/7. In the rare event that both physicians are out of town, coverage will be provided by another trusted concierge internal medicine practice with whom we have a professional relationship.

Do I still need health insurance if I enroll in your practice?

Yes. Our practice complements, rather than replaces, your existing health insurance. We encourage maintaining your Medicare or other insurance plans, as well as participating in FSA or HSA plans if eligible.

Will you still bill my insurance for office visits?

Yes, we will bill your insurance directly for office visits. If your plan requires a co-pay, it will be collected at the time of service.

What if you are not a provider on my insurance plan?

If we are out-of-network with your insurance plan, we can refer you to in-network providers for consultations, diagnostic tests, or hospitalizations. Our office fees are adjusted to help offset any additional costs you may incur.

Is the annual fee tax-deductible, and can I use my HSA account?

The annual fee may qualify as a medical expense for tax purposes, and you may be able to use your HSA account to cover the membership fee. We recommend consulting your tax advisor for specific guidance.

What about lab tests, x-rays, specialist fees, and hospitalizations?

Your annual fee covers membership in the practice and the benefits listed on the Highlights & Details page. Any additional procedures or services not provided in our office will be billed separately by the service provider.

What should I do in an emergency?

For life-threatening emergencies, call 911 immediately. Afterward, contact our office or ask the hospital staff to notify us. For non-urgent issues, please contact our office first.

What if I become ill while traveling?

In a life-threatening emergency, seek immediate care and notify us as soon as possible. For minor concerns, contact us directly. We can prescribe most medications to locations nationwide. If you visit an emergency room or urgent care, ask the attending physician to coordinate with us.

What if I need to see a specialist or surgeon?

We will assist in identifying the most suitable specialist and coordinate the referral process. We will ensure that relevant medical information is shared with the specialist in advance.

Do I have to pay the annual fee even if I dont use your services?

Yes, the annual fee secures your membership in the practice, regardless of your current health status. We encourage you to take full advantage of the services available throughout the year.

What happens if I move out of the area?

If you relocate and need a new primary care physician, your annual fee will be refunded on a pro-rated basis, depending on whether your annual wellness evaluation has been completed. Upon receiving a signed release, we will transfer your medical records to your new physician.

Can I enroll later?

As this is a membership-based practice with limited enrollment, once we reach capacity, a waiting list will be established. Maintaining a limited patient roster allows us to provide the highest level of personalized care.
